Order by Phone
Call our helpful sales team at 1800 762 328 to place your order. Please have your order details ready. Note that phone orders require payment with a major credit card, and Cash on Delivery (COD) is not accepted.

Download and print our Order Form. Fill it out completely with your jewelry choice, preferred engraving, payment, and shipping information. After reviewing it for completeness, mail the form to: Universal Medical ID, P.O. Box 108, North Ryde BC NSW 1670.
